Membership Change Form

Make changes to your membership by filling out the form below. Any submissions sent on or after the first will be processed for the following month.

Cancellation & Freeze Policy: Cancellation and freeze requests must be submitted through the appropriate online form on our website at least three days before your next billing date. Requests are not accepted by phone, email, or at the front desk. All membership freezes begin on the first day of the month.

The Freeze fee is $8 per month per member – Maximum Free Fee amount $16
